Method 3.0 expanded continuity measures

Added exact service destinations, public practitioner records, NPI visibility, and method-compatible continuity counts.

Immutable method 2.0 edition retained

Published the checksum-bound 2026-08-10 aggregate edition. Its historical bytes remain unchanged.
